Vital Records Order Form

Records for the following dates can be requested from the State Archives.
Births and Marriages:   1853-1924 (statewide index available)
Deaths:   1853-1974 (statewide index available)
Municipal Registrations:   Pre-1853 (microfilm copies only, original manuscripts maintained
  at the local level)

Please do not include social security numbers in this request.

The State Archives maintains open vital records transferred from the Department of Health for birth and marriage records more than 101 years old and death records more than 51 years old that occurred in the state of Rhode Island.

For certified pre-1853 vital records, contact the corresponding city or town clerk.

Requests for state filings of births and marriages less than 100 years old, and deaths less than 50 years old, should be directed to:

Do you need a Certified Copy?

Certified copies are only necessary when a legal document is required, such as applying for dual citizenship, handling probate affairs, or for joining genealogical and historical groups and societies.

General genealogical family history research does not require a certified copy. Free vital record copies are available online 24/7 in the Digital Archive here.
